If it's in your character's POV (which I would assume to be first person), referring to him by his first name would be acceptable - especially if your character has a negative opinion of him.
If, however, your character is directly speaking to him, you should refer to him by his last name or...
I actually find '?!' useful for portraying a kind of enthusiastic irony. I'm not entirely sure whether it's actually used in literature, but like you, I imagine not.
I go with the time limit most of the time. The character having to race against the clock to accomplish some kind of task definitely gets ME on the edge of my seat.
