Free Copies for Reviewers

When providing free copies to reviewers for Amazon, goodreads, etc., is it customary to provide a Kindle version, a PDF, or what? Is there a certain etiquette for this? Is it assumed the writer will provide the Kindle or Nook version, or is it entirely up to the reviewer? Does the writer simply ask the reviewer which version he'd prefer? How does the protocol for this typically work? I want to look professional for any reviewers so I want to do this right.

It's up to the reviewer, generally. As they have to provide the review it's best to give them what they want :)

I generally provide a Smashwords coupon, that works for either epub, mobi, pdf.

Many reviewers have a blog. It is good policy to read it. Generally the reviewer provides instructions how they want to receive your book.
